Visiting Hospice Nurse
In order to assist our patients with being comfortable at home, we will be providing nurses who will conduct regular visits. During these visits, they will assess your needs and create a care plan that will relieve you of the symptoms of the illness you have. It is our goal to ensure that you and your loved ones are able to spend as much time with each other as possible. We can also adjust the care plans depending on what your primary care provider recommends and what your wishes are.
Bereavement Support
When your loved one passes away, it can be difficult to deal with the grief and aftermath alone. Knowing this, we are here to provide our help. Our bereavement support services ensure that you are not alone during the most difficult time of your life. We will be here beside you to give you all that you need and much more. Don’t hesitate to ask us how we are able to assist you. We are more than happy to provide you with answers
Hospice Care Coordination
In order to provide you and those you love with the best care possible, we will coordinate with your primary care providers. By doing so, we will be able to cover all of your needs and understand what you need to be more comfortable and relaxed each day. We always strive so that you are able to receive only the best from us and your healthcare providers.
Pain Management
One of the most important aspects of hospice care is being able to provide our clients with pain management services. Due to the symptoms of the illness, you may experience frequent pain and discomfort. We can help you with that. Our staff members will work with you so that you won’t have to feel any pain or discomfort during your day-to-day life.
